Job summary

Moms In Motion in Virginia is seeking a Service Facilitator (SF) Case Manager to support families navigating Medicaid Waiver programs. You will visit clients at home, coordinate plans of care, and help ensure safety, health, and independence.

Requirements include 2+ years of relevant experience, a valid driver's license, strong communication skills, and the ability to work remotely from a laptop. Must reside in VA with local travel to client homes.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience valued.
• Proficient with email, docs, spreadsheets, portals, and video calls.
• Valid driver’s license and reliable vehicle.
• Strong written and spoken communication.
• Background check and 2 professional references.
• Bonus: Person-Centered Thinking/Planning training.

Responsibilities

• Hit the road locally to visit clients in their homes.

Based on the position’s expectation of completing a minimum of 125 visits per month, anticipated annual earnings generally range from $40,000 to $55,000. Actual earnings will vary based on the number and type of visits completed.
