Job summary

Moms In Motion is seeking Service Facilitators to connect with families, helping them navigate Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT) and ensuring those you serve can live safe, healthy, and independent lives.

If you’re compassionate, organized, and enjoy making a difference in your community, you’ll fit right in. You’ll visit clients in their homes, build relationships, and work remotely from a laptop while traveling locally.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience matter more.
• Proficient with email, docs, spreadsheets, and video calls.
• Ability to pass background check and provide two references.
• Bonus: Person-Centered Thinking/Planning training.

Responsibilities

• Visit clients in their homes on a local travel basis.
• Guide families through Virginia waiver programs.
• Write care plans and assessments that drive results.
• Troubleshoot authorizations, timesheets, and related tasks.
• Build lasting relationships with families based on trust.

What You’ll Do

• Hit the road (locally!) to visit clients in their homes.
• Be the go-to guide for families navigating waiver programs.
• Write up plans of care and assessments that actually make a difference.
• Troubleshoot challenges like service authorizations, timesheets, and more.
• Build lasting relationships with families built on respect, patience, and trust.
• Work remotely from your laptop/tablet.

What We’re Looking For

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required (we care more about heart and experience).
• Tech-savvy enough for email, docs, spreadsheets, portal navigation and video calls.
• A valid driver’s license & reliable vehicle (no client transport).
• Great communication—both written and spoken.
• Able to pass a background check + provide 2 professional references.
• Bonus points if you’ve got Person-Centered Thinking/Planning training.

Based on the position’s expectation of completing a minimum of 125 visits per month, anticipated annual earnings generally range from $40,000 to $55,000. Actual earnings will vary based on the number and type of visits completed.
