Residential Services Incorporated in Indiana seeks a Nurse Coordinator to manage medical services for individuals, ensuring accurate documentation and timely communication with physicians, DSPs and families.

The role requires an active LPN/RN license, 2–4 years of nursing experience, and a valid driver’s license with vehicle insurance. Knowledge of Medicaid waivers, 460 IAC guidelines, and RSI policies is essential for success.

Qualifications

  • Active nursing license (LPN or RN) and current license in Indiana.
  • 2–4 years nursing experience, preferably with individuals with developmental disabilities.
  • Free from communicable diseases and able to provide reliable transportation.
  • Current automobile insurance for vehicles registered in Indiana.
  • Clean criminal history and OIG record, no aide registry findings.
  • Ability to document in a timely and accurate manner per 460 IAC and RSI policy.

Responsibilities

  • Coordinate health and medical services for individuals.
  • Ensure documentation complies with 460 IAC 6 and RSI policies.
  • Manage seizure and health-related incidents as they arise.
  • Contribute input into evaluations and disciplinary actions for DSPs.
  • Monitor and document nutritional needs and meal-related supports.
  • Coordinate annual and routine exams, referrals, and physician orders.
  • Communicate with pharmacies regarding medication orders.
  • Supervise DSPs and PDs in medication administration and MAR use.
  • Train staff on medication administration (Core A and B).
  • Collaborate with families, physicians, and state/federal personnel as needed.

Job description

Position is responsible for the provision of medical coordination services to manage health care needs of individuals. Position requires attention to detail, accurate, thorough and timely documentation, a team mentality and a commitment to the individuals that RSI serves.

Reports To: Registered nurse (Nursing oversight)

Executive Director (Operational, programmatic issues)

Experience and Qualifications:
  • Current Nursing License (Licensed Practical Nurse or Registered Nurse.
  • 2-4 years of nursing experience, preferably involving individuals with developmental disabilities.
  • Free from communicable diseases.
  • Must have a working vehicle
  • Current automobile insurance (for vehicles registered in State of Indiana)
  • Clean criminal history and OIG record
  • No findings on any aide registry
Competency/Skill Requirements:
  • Strong nursing and supervisory skills
  • Good communication and organizational skills
  • Ability to document in a timely and accurate manner according to principles of documentation
  • Excellent interpersonal and communication skills
  • Ability to accomplish tasks with minimum supervision in a timely manner
  • Strong observation, organizational and problem-solving skills
  • Knowledge of Medicaid and Medicaid waiver
  • Knowledge of health care coordination services as defined in 460 IAC
  • Ability to interact successfully with a variety of individuals and family members, direct care staff member, physicians and state and/or federal personnel.
  • Basic knowledge of word processing and Word software
Key Responsibilities and Duties:
  • 1. Coordinate health and medical services of individuals.
  • 2. Documentation must be in compliance with 460 IAC 6 (See attached).
  • 3. Seizure and health related incident management.
  • 4. Giving input into DSP evaluations and disciplinary action of DSP’s who fail to pass medication to individuals per 460 IAC and RSI company policy.
  • 6. Monitor and document individuals’ nutritional needs i.e. special dining needs, swallowing difficulties, aspiration risk, needs of adaptive equipment and positioning during meals
  • 7. Coordination of health care i.e. annual exams, routine examinations as ordered by physician, identification and treatment of allergies as ordered by physician, referrals to specialists, communication with individuals primary care physicians and specialists, documenting and following physicians orders
  • 8. Communication with pharmacy regarding individuals’ medications as per physician’s orders.
  • 9. Provide supervision of DSP’s and PD’s regarding medication administration and MAR implementation.
  • 10. Training of all staff regarding medication administration (Core A and B).

Position Physical Requirements:

Position requires the ability to walk, sit, stand, lift, push and pull up to 50 pounds unassisted and 350 pounds with assistance, squat, bend, reach and stretch
