Coordinator - Supervised Group Living Winamac/Logansport

Responsibilities
  • Advocacy – Assist clients with personal goals, help clients participate in activities of their choice, and serve as a liaison for clients as needed.
  • Health Maintenance – Provide care to promote good health, administer medications, record health events, provide First Aid and CPR, and keep medical appointments in the Client Files.
  • Personal Care – Support clients with eating, grooming, dressing, bathing, toileting, and mobility including lifting clients from seated or lying positions and assisting with walking.
  • Life Skills Development – Support clients to develop living skills such as cooking, money management, household shopping, laundry, personal hygiene, and record progress on skill development.
  • Behavior Development – Serve as a role model and mentor, use approved behavior support plans, provide training to other staff, and carry out approved physical intervention techniques when necessary.
  • Operational Support – Maintain cleanliness of the home, provide transportation for clients to activities, follow procedures for handling money, and complete tasks beyond the client’s abilities.
  • Professional Behavior – Act in a professional manner, complete all required training programs at least 24 hours annually, and attend required meetings.
  • Supervisory Responsibility – Train new and current staff, provide client‑specific training, behavior support plan training, risk plan training, and report staffing issues to Human Resources.
  • Scheduling – Schedule staff to cover hours in the home and stay within budget expectations.
  • Records – Verify DSP time records for accuracy, minimize overtime, and submit documents to payroll; on‑call responsibilities and 24‑hour availability are required.
  • Financial Responsibility – Balance and reconcile client petty cash monthly, manage the household budget, and maintain transaction records for proper reporting.
Qualifications
  • Must have a High School Diploma or GED.
  • Must have a valid Indiana Driver’s License, valid auto insurance, and pass required background checks.
  • Three (3) years’ experience working with persons with developmental disabilities.
  • Three (3) years of supervisory experience preferred.
  • Knowledge of state and federal rules and regulations pertaining to the program area preferred.
Working Conditions
  • Possible exposure to blood borne pathogens.
  • Possible exposure to profanity, violent or offensive behavior from clients.
  • Variable hours depending on job location; Peak Community Services operates 24/7, 365 days a year.
Physical Demands

Applicants must be able to lift up to 70 lbs., walk, bend, crouch, kneel, twist, push, pull, squat, and turn; demonstrate normal hearing and vision with corrective devices as needed; endure extended periods of driving and exposure to dust, gas, and fumes; exhibit the ability to sit in a normal seated position for extended periods; and pass a TB test if required.
