Job summary

Montgomery County Board of Developmental Disabilities Services is seeking a Service & Support Administrator II to coordinate individualized services and oversee implementation for individuals with developmental disabilities. The role emphasizes person-centered planning, interagency coordination, and ensuring access to required supports.

The SSA II position offers a Monday–Friday schedule with telework options, a comprehensive benefits package, and the chance to contribute to meaningful community

Qualifications

  • Bachelor’s degree or graduate-level degree from an accredited college or university required.
  • Ability to obtain SSA certification, ODPP certification, and AAI certification.
  • Must maintain a valid driver’s license and automobile insurance while operating a vehicle for company business.

Responsibilities

  • Case management reporting to Service & Support Supervisor.
  • Coordinate and facilitate annual Ohio Individual Service Plan (OISP) meetings.
  • Develop OISP with outcome statements and service actions addressing residential, employment, health, and daily living needs.
  • Complete OISP meeting minutes and related checklist requirements.
  • Provide training to providers on OISP services and outcomes.
  • Document SSA services and efforts with TCM case notes.
  • Track Level of Care and Prior Authorization approvals.

Compensation:The starting pay for this position is $24.46/hour and $50,876.80/year based on a 40-hour work week (or lateral move). *Consideration for placement within the salary band will be based upon direct and/or related work history.

Join our team as aService & Support Administrator II (SSA II)and become the primary point of coordination for the individuals we serve. In this role, you will facilitate person-centered planning, oversee service implementation, and ensure individuals receive the support they need to live fulfilling lives.

Key Responsibilities:

  • Case Management position that reports to the Service & Support Supervisor and promotes and supports the Board mission/vision by providing mandated service and support administration services and serves as the primary point of coordination and interdisciplinary team lead to individuals with developmental disabilities.
  • Schedules/coordinates and facilitates annual Ohio Individual Service Plan (OISP) meetings and possible revision OISP meetings on behalf of individuals served.
  • Uses person-centered planning assessment and personal discovery tools to develop an OISP with outcome statements, action steps, and services that addresses the preferences of the individual and his or her residential, employment, vocational, transportation, health, daily living, and financial needs, etc.
  • Complete OISP meeting minutes and other meeting checklist requirements.
  • Provides training to providers on the OISP services and outcomes, and disseminates OISP packets to individuals, guardians/families, and applicable service providers.
  • Provides crisis/emergency assistance and/or problem-solving assistance in routine and non-routine situations to ensure that necessary services are provided; documented and reviewed administratively as applicable.
  • Assists individuals with the provider selection process and interviews as requested.
  • Document SSA services and efforts by completing Targeted Case Management (TCM) case notes.
  • Conducts continuous reviews to ensure OISPs are developed and implemented in accordance with the Service and Support Administration Rule.
  • Completes Level of Care assessments for all Level 1; Individual Options (IO); Self-Empowered Life Funding (SELF) Waiver recipients prior to stated deadlines to initiate or ensure waiver eligibility and continuation of applicable waiver funding.
  • Establishes waiver and other funding source budgets and facilities funding levels to be within the Ohio Developmental Disabilities Profile (ODDP) funding range.
  • Completes ODDP, Acuity Assessment Instrument (AAI) assessments, and Prior Authorization packets as applicable.
  • Enters individual’s demographic information into the agency database within established timeframes and updates the information accordingly to ensure accuracy.
  • Tracks Level of Care (waiver) and Prior Authorization approvals, if applicable.
