Forward Learning Youth & Young Adults (FLYY) is seeking a full-time Service Facilitator/Mental Health Provider to support clients in the Comprehensive Community Services (CCS) program. The role combines case management with teaching mental health skills for youth and young adults ages 6-35.

The position requires delivering trauma-informed, culturally sensitive care, coordinating health services, housing, and education supports, and traveling to clients in homes, schools, or communities

Qualifications

Job Summary: Full time Service Facilitator/ Skills Coach
Job Description:

Forward Learning Youth & Young Adults (FLYY) is seeking a full-time Service Facilitator/Mental Health Provider to provide services for clients enrolled in the Comprehensive Community Services (CCS) program. This role requires someone to work as both a Service Facilitator (case manager) and to teach mental health related skills for individuals in the age range of 6-35.

The Service Facilitator portion of the position provides a wide range of client-centered case management services and works with participants in reaching their mental health recovery goals. Services provided may include: the coordination of mental and/ or physical health care, assistance with housing, money management, medication management, coordination of substance abuse treatment, coordination of vocational and/ or education support, and assistance with activities of daily living.

The Mental Health Provider portion of the position includes providing a continuum of mental health services to a diverse consumer population, including adolescents and young adults with a history of trauma. This position requires a strong foundation in trauma informed care for adolescents and a willingness to be flexible about therapeutic approach and treatment. The position focuses on meeting with youth and young adults to teach coping and life skills, emotional regulation techniques and independent living skills. The position requires travel to most clients in their home, school or in the community.

Position Responsibilities:
Direct Service
  • Utilize effective strategies to engage participants, enhance motivation, and encourage meaningful, collaborative and strengths-based goal planning and progress.
  • Provide assessment, crisis planning and system-wide service facilitation to assertively respond to the urgent and emergent needs of enrolled participants.
  • Provide screening and assessment, service linkage, service facilitation and other allowable and appropriate services on the CCS service array in a manner that is trauma-informed, culturally relevant, and non-stigmatizing.
  • Develop comprehensive individualized assessments and recovery plans and other related paperwork.
  • Assist the participant with identifying and establishing natural and professional supports in the community.
  • Maintain contact with participants to a degree sufficient to support effective engagement as well as monitor and assist progress.
  • Coordinate recovery team meetings at least every six months.
  • Travel to and teach coping and life skills to participants ages 6-35, coordinate with their CCS team and family as appropriate.
  • Complete paperwork for each client interaction in a professional, timely manner.
Program Development & Implementation
  • Evaluate progress in reaching recovery goals and make recommendations for higher or lower levels of care as appropriate.
  • Consult regularly with supervisor, colleagues and team members regarding participants’ needs and progress utilizing a team approach.
  • Plan, attend and participate in in-service training, conferences and outside training programs as authorized and directed.
  • Provide case-related backup services to colleagues as needed.
Required Qualifications

  • Proficient computer skills, specifically in current versions of Microsoft Word, Excel, Google Drive programs; and willingness to learn new skills as needed or required.
  • Ability to pay attention to detail and a commitment to quality and the timely completion of duties and paperwork.
  • Demonstrated ability to work as a member of a team.
  • Ability to be self directed with the workload and proactive about engaging clients and team members.
Preferred Qualifications
  • Bilingual in Spanish language (in addition to English)
  • Knowledge of Dane County’s child and adult community resources, including evidence-based mental health treatment services
  • Experience working as a case manager or CCS service facilitator or Individual Skills provider
  • Specialized knowledge/experience providing transition services to youth and young adults
  • Current Comprehensive Community Services (CCS) certification
  • Experience providing home- and community-based services
  • Demonstrated flexibility, initiative, and creativity
  • Basic knowledge of effective strategies, such as Motivational Interviewing, Dialectical
  • Behavioral Therapy, Assertive Community Treatment and Cognitive Behavioral Therapy, etc.
  • Knowledge of Dane County Juvenile Court and Child/Adult Protective Service Systems
  • Knowledge of substance abuse issues
Physical Demands

This position requires a combination of sitting, standing, walking, bending, and occasional lifting of up to 35 pounds. The employee must be able to communicate effectively through verbal and written means and respond appropriately in crisis situations. Services may be provided in a variety of community-based settings, where environmental conditions and noise levels may vary. The position may also require driving within the community. The regular schedule is Monday through Friday with some flexibility based on program needs. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Wage: $70,800 to $90,000 based on degree and experience.

We offer compensation for both billable session time and non-billable administrative tasks, with an expectation that at least 65% of work hours are dedicated to billable client services. The compensation range for Master’s level clinicians is $37-47 per hour for billable time and $30-$35 per hour for administrative time. Employees working between 40 hours per week can expect to earn a gross annual income ranging from $70,800 to $90,000, based on experience and number of billable hours per week.

Job Type: Full-time

Full-time mental health professionals at FLYY work between 30 to 40 hours per week and are able to build a schedule that works for them.
