Forward Learning Youth & Young Adults (FLYY) is seeking a full-time Service Facilitator/Mental Health Provider to support clients in the Comprehensive Community Services (CCS) program. The role combines case management with teaching mental health skills for youth and young adults ages 6-35.

The position requires delivering trauma-informed, culturally sensitive care, coordinating health services, housing, and education supports, and traveling to clients in homes, schools, or communities

Qualifications

• Bachelor’s or Master’s Degree in social work, psychology, counseling, or a closely related field.
• Minimum of 2 years relevant experience working with individuals with significant mental health challenges.
• Must pass a Wisconsin caregiver background check and criminal history screening.
• Commitment to strength-based, trauma-informed, recovery-focused programming.
• Ability to drive self and others with mileage/travel reimbursement.
• Daily access to a reliable vehicle with valid license and insurance.

Responsibilities

• Engage participants with strengths-based goal planning and progress updates.
• Provide assessment, crisis planning, and service facilitation within CCS.
• Deliver trauma-informed screening, assessment, linkage, and services.
• Develop comprehensive individualized assessments and recovery plans.
• Assist participants in identifying natural and professional supports.
• Maintain contact to support engagement and monitor progress.
• Coordinate recovery team meetings at least every six months.
• Travel to participants to teach coping and life skills (ages 6-35).
• Complete paperwork for each client interaction promptly.
