The Service Development Representative is responsible for developing and maintaining referral relationships within the community and serving as a liaison between the facility and external providers to ensure awareness of services and continuity of care.
This role collaborates with the Medical Director, Program Administrator, clinical staff, and community partners to identify educational needs, develop an annual education calendar, and enhance community understanding of mental health services. The position also supports case management–related functions within the community and facilitates smooth transitions across levels of care.
