Job summary

Sheppard Pratt is seeking a direct care professional for the Residential Rehabilitation Program. You will assess and monitor client needs, develop service plans, connect clients to community services and resources, and advocate for clients as necessary.

Additional responsibilities include enrollment and orientation, coordinating medical appointments, transportation, housing, money management, and medication management plans.

Qualifications

• High school diploma or GED with 1 year related experience.
• Bachelor's degree preferred.
• Willingness to obtain additional certifications and trainings as required.
• Driver's license with 3 points or less and access to an insured vehicle.
• Availability to work days, nights, weekends, and holidays.

Responsibilities

• Assess and monitor client needs; develop service plans; connect clients to community services and resources; advocate for clients.

Schedule: Monday through Friday, 10am to 6pm | Job Location Type: In Person

Residential Rehabilitation Program services are designed for individuals with serious mental illness who have difficulty maintaining housing. Our team helps individuals obtain housing, employment, mental health services, and more.

What to expect.

This is a direct care opportunity to provide support for mental health treatment. You will assess and monitor client needs, develop service plans, connect clients to community services and resources, and advocate for your clients as necessary.

• Assisting clients through program enrollment and orientation.
• Coordinating client medical appointments, transportation, housing, money management, etc.
• Developing and assisting with client medication management plan.

This position has a flat pay rate of $20.34/hr.
