Job summary

Sheppard Pratt in Catonsville, MD, is seeking a Service Coordinator for the Residential Rehabilitation program. You will assess client needs, develop service plans, coordinate access to housing, health services, and community resources, and advocate for clients to secure entitlements.

The role is full-time, in-person with a fixed schedule, and requires a high school diploma or GED (bachelor’s preferred) plus a valid driver’s license.

Qualifications

• High school diploma or GED required with 1 year of related experience.
• Bachelor’s degree preferred and willingness to obtain additional certifications.
• Driver’s license with 3 points or less and access to insured vehicle.

Responsibilities

• Assess and monitor client needs and develop service plans.
• Connect clients to community services and resources and advocate for them.
• Assist with program enrollment and orientation.
• Coordinate appointments, transportation, housing and money management.
• Develop and assist with client medication management plan.
• Maintain accurate documentation and coordinate discharge planning.
