Job summary

Crestwood Behavioral Health, Inc. is seeking a Service Coordinator (Mental Health Case Manager) to assess clients, coordinate the treatment plan, and work with the interdisciplinary team to ensure continuity of care.

The role is based in California and includes full-time hours with a Sunday-Thursday AM schedule. Qualified candidates will have two years of mental health experience, a relevant bachelor’s degree (or substitution), and knowledge of DSM-5 and recovery principles.

Qualifications

• Two years of full-time experience in a mental health setting.
• A bachelor’s degree with a major in psychology, social work, or behavioral sciences may substitute for the two years of full-time work experience.
• Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.

Responsibilities

• Assess clients to identify problem areas necessitating treatment.
• Coordinate interdisciplinary team meetings and discharge planning.
• Oversee overall treatment plan implementation with nursing staff to ensure continuity of care.

Service Coordinator (Mental Health Case Manager)

Job Duties:

The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments. The Service Coordinator (Mental Health Case Manager) also coordinates the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. In this capacity, the Service Coordinator (Mental Health Case Manager) coordinates the overall treatment plan implementation for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.

Schedule:

• Full-Time: AM (Sunday - Thursday)

Qualifications:

• Two (2) years of full-time experience or part-time equivalent in a mental health setting.
• A bachelor’s degree with a major in psychology, social work, or behavioral sciences may be substituted for the two (2) years of full-time work experience or its part-time equivalent.
• Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
• Familiarity with regulatory standards.

Crestwood Behavioral Health Center Solano’s clients participate in Dialectical Behavior Therapy (DBT), and Wellness Recovery Action Plan (WRAP) treatment model, a self-help recovery system designed to decrease symptoms, increase personal responsibility, and improve the quality of life.

The salary range listed below represents the minimum and maximum base pay per hour at the time of posting. Final salary offered to the candidate selected for the position will be based on factors including but not limited to candidate's skills, experience, licensure, and program acuity.

Pay Range:: $23.60 USD - $26.06 USD
