Job summary

Crestwood Behavioral Health, Inc. is seeking an On-Call Service Coordinator (Mental Health Case Manager) to assess clients, coordinate interdisciplinary planning, and guide treatment planning on the Angwin campus in California.

You will work with the treatment team and nursing staff to ensure continuity of care, discharge planning, and implementation of comprehensive plans tailored to each client’s recovery journey.

Qualifications

• Two (2) years of full-time experience in a mental health setting.
• Bachelor’s degree in psychology, social work, or behavioral sciences may substitute for the two (2) years of full-time work experience or its part-time equivalent.
• Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
• Familiarity with regulatory standards.

Responsibilities

• The Service Coordinator assesses clients to identify problem areas needing treatment and completes assessments.
• Coordinates the interdisciplinary team meetings and discharge planning efforts among the treatment team.
• Coordinates the overall treatment plan implementation for the client on the unit, working closely with nursing staff for continuity of treatment.
• Collaborates with nursing and other staff to ensure accurate and timely care coordination.

Title: On-Call Service Coordinator (Mental Health Case Manager)

Job Duties

The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments. The Service Coordinator (Mental Health Case Manager) also coordinates the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team. In this capacity, the Service Coordinator (Mental Health Case Manager) coordinates the overall treatment plan implementation for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.
