A community-focused behavioral health organization is seeking a Service Coordinator (Mental Health Case Manager) in Chula Vista, California. The ideal candidate will assess clients' needs and coordinate treatment efforts among a multidisciplinary team. Applicants should have at least two years of experience in a mental health setting, work with DSM-5, and have relevant qualifications. Service Coordinator (Mental Health Case Manager)

Responsibilities

• Under the supervision of the Program Director, assess clients to identify individual problem areas necessitating treatment and complete assessments.
• Coordinate interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team.
• Coordinate overall treatment plan implementation for the client on the unit, working closely with nursing staff to ensure continuity of treatment efforts.

Schedule

• Full-Time: PM

Qualifications

• Two (2) years of full-time experience or part‑time equivalent in a mental health setting.
• A bachelor’s degree with a major in psychology, social work or behavioral sciences may be substituted for the two (2) years of full‑time work experience or its part‑time equivalent.
• Demonstrated knowledge of DSM‑5 and psychosocial rehabilitation and recovery principles.
• Familiarity with regulatory standards.

Salary

Pay Range: $25 USD – $27 USD per hour.
