• Experience with case management and coordinating services is expected.

Responsibilities

• Interview residents to assess needs and barriers.
• Refer residents to supportive services and resources.
• Develop individual or family case plans and coordinate resources.
• Enter case notes in ETO and related databases.
• Coordinate events and programs with community partners.
• Prepare reports and communicate with the RS team.
