Qualifications

• Bachelor’s degree in human services related field
• Two years of work experience with developmental disabilities and/or case management
• Ability to communicate effectively, verbally and in writing

Responsibilities

• Provides service coordination, advocacy, and support for assigned individuals and families.
• Partners with individuals and families to identify strengths, preferences, needs, and desired outcomes.
• Facilitates development, implementation, and monitoring of IFSPs/IPPs and related plans.
• Coordinates and monitors services to promote developmental progress and independence.
• Advocates for access to public, educational, and community-based services.
• Maintains required documentation and responds to urgent needs per policies.
• Collaborates with Clinical Services and interdisciplinary team members.
