Qualifications

• Bachelor's degree in a human services-related field.
• Minimum of two years of case management experience with ID/DD.
• Ability to communicate effectively, verbally and in writing.
• Must have a valid CA driver’s license, reliable vehicle, and be willing to travel.

Responsibilities

• Coordinate service coordination, advocacy, and support for individuals and families.
• Develop, implement, and monitor individualized plans.
