Alta California Regional Center, Inc. is seeking a Service Coordinator to provide case management and advocacy for individuals with developmental disabilities in the Sacramento area.

Duties include assisting clients and families with gaining access to transportation, training, education, medical care, and community supports, and coordinating with vendors for certifications and referrals. The role requires a Master’s or Bachelor’s degree in Social Work or a related field, strong communication

Qualifications

• Master’s or Bachelor’s degree in Social Work or a related field.
• Excellent written, verbal, and interpersonal communication skills.
• Strong time management and organizational skills.
• Valid California driver license and automobile liability insurance.
• One year of professional social work experience preferred.
• Russian speakers preferred.

Responsabilités

• Provide case management and advocacy for persons with developmental disabilities.
• Assist clients and families in obtaining supports and services including transportation, vocational training, day programming, education, medical care, and independent living training.
• Develop and monitor Individual Program Plans and coordinate with vendors and service providers.
