The Adult Unit Service Coordinator delivers service coordination and advocacy to adults (22+) with developmental disabilities, partnering with individuals, families, and providers to assess needs, coordinate services, develop IPPs, and promote health, safety, independence, and quality of life.

Qualifikationen

• Bachelor's degree in a human services-related field.
• Strong verbal and written communication skills.
• Ability to prioritize and manage multiple assignments while meeting timelines.

• Provide service coordination and advocacy to adults (22+) with developmental disabilities.
• Develop, implement, and monitor Individual Program Plans (IPPs) in compliance with DDS and CVRC requirements.
• Coordinate supports and resources to promote independence, safety, and community participation.
• Document case management activities daily and maintain accurate records.
• Collaborate with clinical services, providers, families, and community partners.
