Job summary

Sheppard Pratt is hiring a Service Coordinator for the Crisis Residential program in Germantown, MD. This in-person, full-time role provides short-term, intensive support to individuals in crisis, coordinating services and advocating for client needs.

You will assess client needs, develop service plans, arrange community resources, and help with transitions and discharge planning. A high level of flexibility to work days, evenings, weekends, and holidays is required.

Qualifications

• High school diploma or GED with 2 years of related experience.
• Bachelor’s degree with 1 year of related experience.
• Willingness to obtain additional certifications and trainings.
• Driver’s license with 3 points or less and access to insured vehicle.
• Ability to work days/nights/weekends/holidays as needed.

Responsibilities

• Assess and monitor client needs and develop service plans.
• Coordinate client medical appointments, transportation, housing, and money management.
• Develop and assist with client medication management plans.
• Maintain entitlements and complete accurate documentation.
• Coordinate transitions and program discharge planning.

Description

Shift: | Job Location Type: In Person

Crisis Residential Program services provide short-term intensive, round-the-clock, personalized support to help stabilize someone having a mental health crisis so they can avoid hospitalization. Our teams collaborate with providers who are already involved in an individual’s treatment and works to link clients to other services offered by Sheppard Pratt or other community providers.

What to expect.

This is a direct care opportunity to support not just the mental health of your clients, but also their overall independence and well-being. You will assess and monitor client needs, develop service plans, connect clients to community services and resources, and advocate for your clients as necessary.

Additional responsibilities will include:

• Assisting clients through program enrollment and orientation.
• Coordinating client medical appointments, transportation, housing, money management, etc.
• Developing and assisting with client medication management plan.
• Obtaining and maintaining entitlements for clients.
• Maintaining accurate and complete documentation.
• Developing and coordinating transitions plans and program discharge

This position has a flat pay rate of $20.34/hr.
