Job summary

Sheppard Pratt in Gaithersburg, MD is seeking a Service Coordinator for the Residential Rehabilitation Program. This full-time, on-site role involves assessing client needs, developing service plans, coordinating with community resources, and advocating for clients to help them obtain housing and services.

You will schedule enrollments, coordinate appointments, and maintain documentation while supporting transitions and discharge planning.

Qualifications

• A high school diploma or GED and 1 year of related experience.
• A bachelor’s degree is preferred.
• The willingness to obtain additional certifications and trainings as required.
• A driver’s license with 3-points or less and access to an insured vehicle.
• Must have availability to work days, nights, weekends, and holidays depending on need of the program.

Responsibilities

• Assess and monitor client needs, develop service plans, and connect clients to community services.
• Advocate for clients as necessary.
• Assisting clients through program enrollment and orientation.
• Coordinating client medical appointments, transportation, housing, money management, medication monitoring, etc.
• Developing and assisting with client medication management plan.
• Developing and assisting with client individual rehabilitation plan.
• Obtaining and maintaining entitlements for clients.
• Maintaining accurate and complete documentation.
• Developing and coordinating transitions plans and program discharge.

Description

Schedule: Monday - Friday, 7:30am to 3:30pm | Job Location Type: In Person

Residential Rehabilitation Program (RRP) services are designed for individuals with serious mental illness who have difficulty maintaining housing. Our team helps individuals obtain housing, employment, mental health services, and more.

What to expect.

This is a direct care opportunity to provide support for mental health treatment. You will assess and monitor client needs, develop service plans, connect clients to community services and resources, and advocate for your clients as necessary.

Additional responsibilities will include:

• Assisting clients through program enrollment and orientation.
• Coordinating client medical appointments, transportation, housing, money management, medication monitoring, etc.
• Developing and assisting with client medication management plan.
• Developing and assisting with client individual rehabilitation plan.

This position has a flat pay rate of $20.34/hr.
