Job summary

ConnectAbility of MN Inc is seeking a Service Coordinator to ensure individuals receive authorized services smoothly, respectfully, and in compliance with Minnesota DHS waiver requirements. This role serves as the primary point of contact for case managers, clients, and vendors, relying on clear communication and problem-solving to coordinate services.

Location is in-office with standard hours, 8:00am–4:30pm, Monday–Friday. Starting pay is $21/hour.

Qualifications

• High school diploma or equivalent.
• 1-2 years of experience in service coordination or human services preferred.
• Ability to interpret Service Agreements and waivers.
• Strong documentation and attention to detail.

Responsibilities

• Coordinate services referred and authorized through Service Agreements.
• Interpret Service Agreements per DHS waiver rules.
• Monitor service status and communicate changes to stakeholders.
• Ensure services align with authorization and timelines.
• Support billing by ensuring services match authorizations.
• Maintain accurate client records in Salesforce.
