Job summary

A community service organization in Vermont is seeking a Service Coordinator to support individuals in their homes and manage various state-required expectations. This role offers a generous benefits package, including medical, dental, and vision insurance, as well as a $500 sign-on bonus after six months. Must have a college degree in Human Services and valid driver's license.

Qualifications

• Must be available M-F 8 am-4 pm, with occasional evenings/weekends.

• Upper Valley Services is a person-centered program.
• Service Coordinators meet at least monthly with the shared living provider and participant in their home to support and manage various aspects of the state-required expectations to ensure a healthy environment that meets the participant’s goals and needs.
• Mix of field work and main office in Randolph.
• Background checks are required.
• Must have valid driver’s license and insured vehicle.

• Thorough initial and ongoing training for Service Coordinators.
• M-F 8am-4pm and occasional evening or weekend availability.
• College degree in Human Services or related field preferred.
• We are offering a generous sign on bonus!! $500 after successfully completing six months and another $500 upon successfully completing one year at Upper Valley Services.
