Howard Center in Burlington, VT is seeking a Service Coordinator - Public Safety to manage and supervise all aspects of service for individuals with intellectual disabilities and/or autism. Working within a clinical team, you will develop, plan, and implement case management, therapy, and community/residential support, focusing on adults with developmental disabilities and significant medical needs.
This exempt role supports aging clients including end of life care.
Manage and supervise all aspects of service for individuals with intellectual disabilities and/or autism spectrum disorder. Work within a clinical team to develop, plan, and implement case management, therapy, and community/residential assistance. The Service Coordinator will specialize in supporting adults with developmental disabilities and significant medical needs. This would support an aging population of clients including end of life care. These clients require many specialized care procedures and medication administration. This Service Coordinator would be in contact with external partners like the hospital, medical teams, families, guardians, shared living providers, internal nurses, etc. Please note while this is the primary focus of this team, you will support clients that do not align with the details above. This role would work collaboratively with leadership and coworkers to offer the best support to clients and service coordinators alike.
