Job summary

Concern Housing is seeking a Service Coordinator to provide linkages with community resources, daily living skills training, counseling and support, documentation, and program-compliance. The Service Coordinator serves as an essential resource and advocate for clients.

Schedule: Sunday - Thursday; 8am - 4pm. Full-Time position located on Jerome Ave., Bronx (St. James Terrace).

Qualifications

• Bachelor's Degree in Psychology, Rehabilitation, Social Work or related mental health discipline.
• Good verbal and written communication skills.
• Ability to make sound judgments regarding client care.

Responsibilities

• Facilitate residents’ linkage with community services (case management, clinical services, day programs, etc.).
• Develop quarterly goal plans and conduct weekly progress meetings; maintain client records and discharge plans.
• Provide individual counseling and support services.
• Assist with housing maintenance and related housing services.
• Provide daily living skills training (medication monitoring, cleaning, budgeting).
• Perform monthly fire safety inspections.
• Attend weekly supervision and staff meetings; maintain agency documentation and trainings.
• Other duties as assigned by the Program Director or Supervisor.

Description

Concern Housing is seeking a Service Coordinator to beresponsible for providing linkages with community resources, daily living skills training, counseling and support, documentation, and compliance with program requirements. The Service Coordinator is an essential resource and advocate for their clients.

Schedule: Sunday - Thursday; 8am - 4pm

Duties and Responsibilities

• Facilitate residents’ linkage with services available in the community (e.g., case management, clinical services, day programs, vocational services and opportunities, community integration, entitlements, etc.).
• Development of quarterly goal plans, functional assessments, goal plan reviews. Weekly meetings with clients to develop skills and review progress. Responsible for all documentation required for client record (e.g., progress notes, discharge plans, etc.).
• Provide individual counseling and support services.
• Housing services including assistance with maintenance issues.
• Provide daily living skills training (e.g., medication monitoring and training, cleaning, money management, etc.).
• Perform monthly fire safety inspections.
• Attendance at weekly supervision and staff meetings.
• Remain current with all Agency required compliance documents and trainings.
• Other duties as assigned by the Program Director or Supervisor.

Qualifications

Bachelors Degree in Psychology, Rehabilitation, Social Work or other related mental health discipline, plus 2 years experience working in a similar setting and/or working with people with mental illnesses. The individual in this position must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care.
