Concern Housing is seeking a Daily Living Skills Counselor to provide life skills training and hands-on assistance to clients in the supported housing program in Middle Island. This role focuses on helping clients develop independence and self-reliance through practical education and guidance.

The position involves collaboration with the case management team, documentation of visits, adherence to policies, and participation in weekly supervision.

Qualifications

• Three years’ experience in a similar setting or with people with mental illness.
• Good verbal and written communication skills; ability to make sound judgments regarding client care.

Responsibilities

• Provide education and skill building across activities of daily living including personal hygiene, living environment maintenance, travel training, money management, food preparation, nutrition, shopping, self-preservation, communication, community integration and wellness self-management.
• Provide individual counseling and support services related to life skills development.
• Collaborate with the case management team; maintain open communication with the case manager on skill assessment and goal planning.
• Develop training tools in coordination with the case manager to address client needs.
• Maintain documentation of visits and activities per agency policy; conduct site visits as directed by Supervisor; adhere to agency policies and attend supervision and staff meetings.

Education

Bachelor's degree in Psychology, Rehabilitation, Social Work or other related mental health discipline

Description

Concern Housing is seeking a Daily Living SkillsCounselor who will be responsible for providing life skills training services directly to clients and is considered a resource in that they should be familiar with the skills and tasks necessary to help consumers learn self-reliance skills. The primary task is to provide hands on assistance to clients to help them to develop the skills necessary to maintain independence in the supported housing program.

Responsibilities include but are not limited to:

• Provide education and skill building assistance across multiple activities of daily living including but not limited to maintaining personal hygiene, maintenance of the living environment, laundering of clothing, travel training, money management, food preparation, nutrition, shopping, basic self-preservation skills, communication skills, community integration, and wellness self-management.
• Provide individual counseling and support services as it pertains to life skills development.
• Work as an integral part of the case management team. Maintain open communication with the case manager regarding skill assessment and goal planning activities.
• Develop training tools and techniques in coordination with case manager to address identified client needs.
• Maintain all documentation pertaining to visits and activities as consistent with agency policy.
• Conduct site visits as needed and directed by Supervisor.
• Adherence to all policies as set forth in Concern’s personnel manual.
• Attendance at weekly supervision and staff meetings.
• Remain current with all Agency required compliance documents and trainings.

Requirements:

Three years’ experience working in a similar setting/and or working with people with mental illness. Bachelor’s degree in Psychology, Rehabilitation, Social Work or other related mental health discipline preferred. The individual in this position must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care.
