Concern Housing in New York seeks a Service Coordinator to link residents with community resources, provide daily living skills training, counseling, and ensure program compliance. The Service Coordinator acts as an essential advocate for clients and accurately documents progress.

Qualifications

• Bachelor's degree in psychology, rehabilitation, social work or related mental health discipline.
• Two years of experience in a similar setting or with people with mental illnesses.
• Strong verbal and written communication skills with good judgment.

Responsibilities

• Facilitate residents’ linkage with community services such as case management and vocational services.
• Develop quarterly goal plans and conduct weekly progress meetings; document client records.
• Provide individual counseling and support services.
• Assist with housing services including maintenance issues.
• Provide daily living skills training (medication monitoring, cleaning, money management).
• Perform monthly fire safety inspections.
• Attend weekly supervision and staff meetings.
• Maintain agency compliance documentation and trainings.
• Carry out other duties as assigned by Program Director or Supervisor.

Concern Housing is seeking a Service Coordinator to be responsible for providing linkages with community resources, daily living skills training, counseling and support, documentation, and compliance with program requirements. The Service Coordinator is an essential resource and advocate for their clients.

Schedule

Sunday - Thursday; 8am - 4pm

Duties And Responsibilities

• Facilitate residents’ linkage with services available in the community (e.g., case management, clinical services, day programs, vocational services and opportunities, community integration, entitlements, etc.).
• Development of quarterly goal plans, functional assessments, goal plan reviews. Weekly meetings with clients to develop skills and review progress. Responsible for all documentation required for client record (e.g., progress notes, discharge plans, etc.).
• Provide individual counseling and support services.
• Housing services including assistance with maintenance issues.
• Provide daily living skills training (e.g., medication monitoring and training, cleaning, money management, etc.).
• Perform monthly fire safety inspections.
• Attendance at weekly supervision and staff meetings.
• Remain current with all Agency required compliance documents and trainings.
• Other duties as assigned by the Program Director or Supervisor.
