Job summary

Concern Housing is seeking a Daily Living Skills Counselor in Middle Island, NY to provide hands-on life skills training and support within the housing program. You will help clients develop independence through practical education on daily activities and community integration.

You will collaborate with the case management team to assess needs, plan goals, and document progress, while maintaining compliance with agency policies.

Qualifications

• Three years’ experience in a similar setting or with people with mental illness.
• Bachelor’s degree in Psychology, Rehabilitation, Social Work or related mental health discipline preferred.
• Good verbal and written communication skills with sound judgment regarding client care.

Responsibilities

• Provide education and skill building across daily living activities including personal hygiene, home maintenance, money management, and meal planning.
• Provide individual counseling and support services for life skills development.
• Work as part of the case management team and communicate about skill assessments and goal planning.
• Develop training tools in coordination with the case manager to address client needs.
• Maintain all documentation of visits and activities per agency policy.
• Conduct site visits as directed by the Supervisor.
• Attend weekly supervision and staff meetings and stay current with required trainings.

Description

Concern Housing is seeking a Daily Living SkillsCounselor who will be responsible for providing life skills training services directly to clients and is considered a resource in that they should be familiar with the skills and tasks necessary to help consumers learn self-reliance skills. The primary task is to provide hands on assistance to clients to help them to develop the skills necessary to maintain independence in the supported housing program.

Responsibilities include but are not limited to:

• Provide education and skill building assistance across multiple activities of daily living including but not limited to maintaining personal hygiene, maintenance of the living environment, laundering of clothing, travel training, money management, food preparation, nutrition, shopping, basic self-preservation skills, communication skills, community integration, and wellness self-management.
• Provide individual counseling and support services as it pertains to life skills development.
• Work as an integral part of the case management team. Maintain open communication with the case manager regarding skill assessment and goal planning activities.
• Develop training tools and techniques in coordination with case manager to address identified client needs.
• Maintain all documentation pertaining to visits and activities as consistent with agency policy.
• Conduct site visits as needed and directed by Supervisor.
• Adherence to all policies as set forth in Concern’s personnel manual.
• Attendance at weekly supervision and staff meetings.
• Remain current with all Agency required compliance documents and trainings.

Requirements:

Three years’ experience working in a similar setting/and or working with people with mental illness. Bachelor’s degree in Psychology, Rehabilitation, Social Work or other related mental health discipline preferred. The individual in this position must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care.
