Job summary

StoryPoint in Minnesota is seeking a Wellness Team Supervisor to oversee day-to-day supervision of care staff in assisted living and memory care, ensuring residents receive personalized assistance.

You will lead a team, coach staff, manage schedules, and monitor care standards, participating in safety checks and ongoing training. Weekend or on-call availability may be required in this role.

Qualifications

• High School Diploma or GED preferred.
• Experience in Healthcare, Geriatrics, or with the elderly preferred.
• 2 years of Leadership Experience.
• CNA/LPN/RN with at least 2 years of related experience or training or equivalent.
• Ability to follow instructions in written, oral, or diagram form.
• Ability to lift/move up to 25 pounds occasionally.

Responsibilities

• Coach and support care staff; monitor performance and conduct corrective actions as needed.
• Perform daily rounds to ensure proper care and adherence to standards and regulations.
• Verify staff hours, manage schedules, and coordinate coverage within budget.
• Lead safety and infection control checks and document compliance.
• Oversee training and onboarding of new staff; respond to emergencies as needed.
• Participate in hiring, disciplinary decisions, and related processes.
• Support community events and family interactions; ensure continual improvement.
