Job summary

Grand Living in West Des Moines, IA seeks a Health & Wellness Assistant to create exceptional daily experiences for residents. You will provide compassionate care, support independence, and deliver personalized service through The Sterling Touch®, assisting with activities of daily living and wellness initiatives.

Responsibilities include assisting with daily living tasks, observing residents, documenting services, and collaborating with care teams to ensure a warm, engaging, and respectful

Qualifications

• High School Diploma or GED required.
• 1+ year of experience providing assistance with activities of daily living preferred.
• 1+ year of experience working with seniors preferred.
• CNA and/or CMA credentials preferred; experience in lieu of licensure considered.

Responsibilities

• Assist residents with activities of daily living including dressing, eating, grooming, personal hygiene, mobility, communication, and toileting.
• Support residents according to personalized care plans while promoting dignity, independence, and well-being.
• Observe residents and communicate changes to the nursing team.
• Build meaningful relationships with residents and families in a warm, engaging atmosphere.
• Participate in community activities, wellness programs, dining, and events.
• Collaborate with Health & Wellness, Culinary, Lifestyle, and Hospitality teams to deliver an exceptional resident experience.
• Ensure documentation for all services provided or declined is accurate.
• Deliver care that is professional, respectful, timely, and personalized.
• Help create an environment where every resident feels valued and cared for.
