Responsibilities

• Provide comprehensive case management and preventative outreach.
• Conduct wellness assessments and follow up on identified needs.
• Coordinate with primary care physicians, hospitals, and community providers.
• Ensure effective communication around status changes and service utilization.
• Follow up on hospitalizations, rehab stays, and ED visits.
• Assist residents and families with transitions to higher levels of care.
• Educate residents and families on health and wellness topics.
• Document all work electronically in the resident management system.
