Qualifications

• Graduate from an accredited school of nursing or equivalent.

• RN licensure in NY or a compact state.

Responsibilities

• Assess and coordinate care across the continuum for complex/high acuity populations.
• Develop and monitor long-term care/service plans with providers and resources.
• Educate members and families on services and benefits.
• Serve as liaison between member, family, physician, and facilities.
• Ensure compliance with state/federal guidelines and maintain member information.
