Fidelis Care - New York is seeking a field Care Manager to perform UAS assessments for the MLTC member population across Schuyler, Steuben, Livingston, and Allegany counties. NY RN licensure is required to qualify for this role, with responsibilities spanning from evaluating complex care needs to coordinating long-term care plans and resources for members and families.

The role emphasizes person-centered care, collaboration with providers, and ongoing updates to care plans.

Qualificações

• RN licensure in NY or Compact licensure required
• Graduate from an accredited school of nursing or a Bachelor's degree; 4–6 years of related experience
• Bachelor's degree in nursing preferred

Responsabilidades

• Evaluates the service needs of the most complex or high risk/high acuity members and recommends a plan for the best outcome
• Develops and continuously assesses long-term care plans/service plans with care management team
• Coordinates between member/family and care team to ensure person-centered care
• Monitors and revises care plans to meet member goals/needs
• Reviews member data to identify trends and improve quality of care per regulations
• Facilitates education to members and families on disease processes and benefits
• Acts as liaison between member, family, physician, and facilities/agencies
• Provides coordination of service authorization for various services
• May perform home/site visits (monthly or more) to assess needs
• Partners with leadership to improve quality of care in a cost-effective manner
• Precepts clinical new hires and guides onboarding for readiness
