Job summary

Fidelis Care - New York is seeking a field-based Care Manager to assess and coordinate medical and support services for complex/high acuity MLTC members in Suffolk County. You will develop personalized care plans, educate families, and liaise with providers to ensure high-quality, cost-effective care.

The role requires NY RN licensure and 4–6 years of relevant experience, with a focus on person-centered care coordination and regulatory compliance.

Qualifications

• RN license in NY is required
• Bachelor's degree in Nursing required; graduate from accredited school of nursing preferred
• 4–6 years of related experience in care management or MLTC settings

Responsibilities

• Evaluates the service needs of the most complex or high risk/high acuity members and recommends a plan for the best outcome
• Develops and continuously assesses long-term care plans with the care management team and identifies providers or resources
• Coordinates between member/family and care provider team to ensure person-centered care
• Monitors progress toward care plan goals and revises plans as needed
• Reviews member data to identify trends and improve quality of care while complying with regulations
• Educates members and families on services, benefits, and options
