Job summary

Amaze Health is seeking a Triage Nurse to act as the first clinical contact for patients, assessing symptoms, determining acuity, and guiding care. This fully in-office role requires strong assessment skills and collaboration with care teams.

Responsibilities include patient assessments, clear communication, accurate documentation, and patient education to support timely follow-up and optimal outcomes.

Qualifications

• Active RN or LPN license in good standing.
• Minimum 5 years of in-person nursing experience.
• Experience in triage, urgent care, or high-acuity settings.

Responsibilities

• Conduct initial patient assessments to evaluate severity and advise on care methods.
• Deliver effective communication with patients and their families regarding symptoms and treatment options.
• Document patient interactions accurately to ensure effective follow-up and continuity of care.
• Collaborate with the multidisciplinary team to optimize patient health outcomes.
• Educate patients about disease management and preventive health measures.
• Utilize clinical judgment to prioritize medical inquiries and facilitate patient placement.
• Ensure a patient-centered experience at all points of contact.
