OhioHealth Corporation is seeking a Social Worker MSW, LISW at our Trauma Services department. You will conduct biopsychosocial assessments, develop treatment plans, provide individual/family/group therapy, and manage discharge planning in a high-stress hospital setting.

Role includes staff education, mentoring, project development, and potential supervision of interns and junior staff, with licensure to practice independently per Ohio regulations.

Qualifications

• Master's Degree: Social Work (Required)
• DL-HC - Driver's License only if in a Home Care department - Department of Motor Vehicles
• LISW - Licensed Independent Social Worker - Social Work Certification and Licensure Board
• LPCC - Licensed Professional Clinical Counselors - American Counseling Association

Responsibilities

• 30% Advanced Clinical Assessment: Interviews, chart reviews, and collaboration to assess biopsychosocial functioning and plan care including discharge planning.
• 30% Advanced Intervention Management: Develops intervention plans using individual, family, or group modalities with continuum of care planning.
• 25% Advanced Population Management: Coordinate clinical and financial outcomes with interdisciplinary teams and external organizations.
• 10% Documentation and Regulatory Compliance: Document assessments and interventions per standards and regulatory requirements.
• 5% Maintains awareness of community resources and facilitates linkages for patients.
