Job summary

FutureSoft Consulting Inc. seeks an experienced Registered Nurse Case Manager for a 13-week travel assignment in Juneau, Alaska. The ideal candidate has recent hospital case management experience, skilled in care coordination, discharge planning, utilization review, and interdisciplinary collaboration.

Candidates must hold an active Alaska RN license and have RN Case Management experience within the last two years. Strong communication and organizational skills are essential.

Qualifications

• Active Alaska RN license (unrestricted).
• Associate Degree in Nursing from an accredited program.
• Current BLS certification.
• Recent RN Case Management experience within the past 2 years.
• Experience in an acute-care hospital setting.
• Strong discharge planning and care coordination skills.

Responsibilities

• Coordinate patient care during hospital stay and transitions.
• Develop and manage individualized discharge plans.
• Collaborate with physicians, nurses, social workers, rehabilitation teams and other professionals.
• Assess clinical, psychosocial, financial, and discharge planning needs.
• Identify barriers to discharge and resolve them.
• Coordinate post-acute services (SNF, home health, rehab, DME, follow-up).
• Communicate with patients and families about care plans and discharge expectations.
• Support safe and timely transitions of care.
• Participate in rounds and care conferences.
• Review progress and update care plans as needed.
• Assist with utilization management and medical necessity reviews.
