Job summary

The Cuyahoga County Board of DD is seeking a current Level 2 Support Administrator to join the Youth & Transition team to mentor new SSAs and provide ongoing guidance. You will facilitate person-centered planning, coordinate services, and support budget development to meet identified needs.

You will also work with providers to ensure timely authorizations, enrollment onto Medicaid waivers when appropriate, and ongoing program review to promote positive outcomes for individuals served.

Qualifications

• Current Cuyahoga DD Support Administrator.
• Ability to drive extensively within and beyond Cuyahoga County; access to reliable transportation.

Responsibilities

• Mentor new SSAs and provide ongoing support to SSAs.
• Assist with problem solving and service coordination across internal and external providers.
• Develop and revise person-centered plans to promote self-determination.
• Develop, authorize, and revise budgets for services based on assessed needs.
• Coordinate enrollment onto Medicaid waivers when appropriate.
• Attend meetings and collaborate with SSA Supervisors to expand practice.
