Job summary

The State of Louisiana Department of Veterans Affairs seeks a Social Worker 5-B at the Southeast Louisiana Veterans Home in Reserve, LA. This role combines social services with leadership to oversee care for Medicare veterans in a 156-bed facility.

You will supervise Case Managers, facilitate admissions, coordinate with healthcare teams, and ensure compliance with policy while serving veterans with compassion and respect.

Qualifications

• Must hold Louisiana LMSW or LCSW license.
• Master's degree in social work required.
• Four years of social services experience required.

Responsibilities

• Supervise Case Manager for medically complex residents in Medicare veteran units.
• Escort and assist veterans to medical appointments.
• Manage admissions protocol including financial affairs, medical records and placement on units.
• Investigate and report resident complaints and incidents; coordinate corrective actions.
• Update veterans face sheets and represent the department at monthly Residents meetings.
