Qualifications

Responsibilities

• Serve as clinical and administrative supervisor for social work staff in extended and specialty care programs.
• Oversee Contract Nursing Home/Adult Day Health, SCI/DPACT social workers, VIST Coordinator, and CLMC/Medical Foster Home teams.
• Develop, implement, and evaluate policies and procedures; manage contracts; supervise data collection and performance measures.
