Responsibilities

• Assess and reassess client functioning levels through interviews and records; determine needed services.
• Develop and implement case plans for safety, permanency and services.
• Coordinate with state agencies, community orgs, and families; monitor progress and adjust plans as needed.
• Conduct home visits, document cases, and prepare reports for courts and agencies.
