Qualifications

• Licensure as a Social Worker or eligible/provisional licensure in Nevada.
• At least one year of professional case management experience in a social work setting.

Responsibilities

• Assess and reassess client functioning using interviews and tools.
• Develop and implement a case plan with intervention strategies.
• Coordinate services with state agencies and community providers.
• Provide ongoing case management, home visits, and progress tracking.
• Organize MDT meetings for protective services planning.
• Document case activities and prepare reports for authorities.
