Responsibilities

• Provide social service activities for patients and families, including planning and consultation with other staff members.
• Assess the patient’s and family’s emotional and social factors to estimate capacity and potential.
• Assist in discharge planning and submit a written summary of services provided for the chart.
• Participate in discharge planning and in-services programs related to patient needs.
