Qualifications

• Licenses and Certifications: LMSW licensed in the state of Texas.
• Preferred: 2 years hospital experience.

Responsibilities

• Screens and evaluates high risk patients for discharge planning needs and consults with the healthcare team.
• Facilitates care conferences to establish safe, effective discharge plans.
• Coordinates referrals and post-acute services to ensure continuity of care.
• Maintains communication with patients, families, and community resources to support transition needs.
• Performs duties involving patient records and coordination with payors and providers.
