Job summary

Veterans Health Administration in Washington, DC seeks a Senior Social Worker for Community Care to coordinate psychosocial care for Veterans involving community-based services and internal VA coordination. Qualifications

• Note: Only education or degrees recognized by the U.S. Department of Education from accredited colleges, universities, schools, or institutions may be used to qualify for Federal employment.

Responsibilities

• Provides comprehensive psychosocial assessment, triage and case management for Veterans currently referred to Specialty Care through Community Care.
• Coordinate with Veterans referred to Community Care and assist with services for treatment, care, admission, discharge planning, and caregiver needs via telephone assessments.
• Maintain a caseload of high-risk Veterans and provide case management and enhanced care.
• Establish and maintain therapeutic relationships with Veterans and caregivers assigned to Community Care.
