Job summary

Qualifications

Responsibilities

• Provide psychosocial assessment, triage and case management for Community Care veterans.
• Coordinate services for treatment, care, admission and discharge planning.
• Act as liaison to ensure veterans connect with required services through the VAMC or community providers.
• Maintain a high-risk caseload and collaborate with multidisciplinary teams.
• Develop policies, procedures, and guidelines to guide Community Care activities.
