Job summary

Piedmont Medical Network is seeking a Social Worker to facilitate care along a continuum, coordinating resources to improve patient health outcomes and appropriate utilization of services. The role focuses on transition needs, risk of readmission, and safe discharge.

The ideal candidate holds a Master of Social Work and active licensure, with preferred 2 years of acute hospital experience. Eligible candidates may have ACM certification; Tenet participates in E-Verify and supports accommodations

Qualifications

• Education: Master of Social Work (MSW) and active licensure in the state(s) served.
• Must be licensed, certified or registered to practice as required by law or regulation.
• Active license for state(s) covered and in good standing.

Responsibilities

• Facilitate psycho-social transition planning, assessment and intervention.
• Assist with adoptions, abuse and neglect cases; assess, intervene and refer to agencies as needed.
• Coordinate or oversee transition plan implementation and care conferences.
• Lead or participate in multidisciplinary team meetings such as Complex Case Review.
• Refer to other departments and communicate with patients and families about the plan of care.
• Perform related duties as required.
