Job summary

Piedmont Atlanta Hospital in Atlanta, GA is seeking a Social Worker to promote patient and family wellness and coordinate complex transition plans in inpatient and ED settings.

You will collaborate with physicians, social work colleagues and community resources to ensure safe discharges and connections to post-acute services. A Master's in Social Work is required; LMSW or other certifications are preferred. This is a full-time position based in a busy hospital environment.

Qualifications

• Master's Degree in Social Work from an accredited program is required.
• Minimum 1 year of experience in an acute or post-acute setting is required.
• LMSW (Licensed Master Social Work) preferred; ACM and CCM certifications preferred.

Responsibilities

• Promote patient and family wellness and improve care outcomes in inpatient and ED settings.
• Develop and manage comprehensive transitional care plans for patients with complex psychosocial needs.
• Collaborate with service teams, physicians, patients, external agencies and families to support transitions.
