Qualifications

• Master of Social Work degree from a CSWE-accredited program and one year of full-time social work experience in a health care setting or equivalent.

Responsibilities

• Provide specialty consultation to health care staff regarding psychosocial aspects of care.
• Provide psychosocial support to patients and families.
• Complete clinical assessments per department standards.
• Collaborate with multidisciplinary team to develop and implement discharge plans.
• Provide care coordination for patients with complex social/medical histories.
• Deliver case management and counseling in specialized programs.
• Refer to hospital and community agencies as required by law.
