Job summary

University of Washington's Harborview Medical Center in Seattle seeks a Social Work Assistant with Inpatient Care Management to support discharge planning, coordinate care across SNFs, Adult Family Homes, transportation brokers and community agencies, and help improve patient flow. The position is per diem with a day shift.

This role requires knowledge of medical conditions, NASW ethics, and strong liaison skills.

Qualifications

• Bachelor of Social Work or BA in behavioral sciences with two years as a Social Work Assistant I or equivalent.
• OR AA in behavioral sciences with four years as a Social Work Assistant I or equivalent.

Responsibilities

• Provide services to Harborview Medical Center patients per policies and NASW Code of Ethics.
• Maintain knowledge of medical conditions, terminology and community resources.
• Promote effective working relationships as an ambassador for the Social Work Department.
• Review referrals for facility placement and submit electronic referrals for provider review.
• Contact potential providers for admission decisions and inform primary MSW as needed.
• Coordinate transportation for Bed Readiness patients with the SNF as needed.
• Be familiar with DSHS and Medicare benefits, eligibility, and related regulations.
• Provide notarial services for patients related to admission or discharge planning.
