Responsibilities

• Psychosocial Assessment - Performs psychosocial assessments considering family, social, and medical history; reports suspicions of abuse to authorities; completes documentation with accuracy.
• Resource Support - Collaborates with the health care team to develop a treatment plan and identify resources for interventions; maintains knowledge of internal and external resources.
• Psychosocial Interventions - Provides counseling, education to patients/families, and self-management support to high-risk cases; coordinates with care teams.
• Interdisciplinary Collaboration - Advocates for patient/family needs within the healthcare system; facilitates communication among care teams.
• Collaboration - Partners with patients and families to ensure safe transition of care and participates in meetings to develop skills.
